Your Role With Us:MSW Licensed Care ManagerUnder the general supervision of the Palliative Care Medical Director and the Clinic Manager, the Palliative Care Social Worker will provide evidenced-based brief solution-focused psychosocial interventions to support the collaborative process of biopsychosocial assessment and treatment, advanced care planning, goals of care discussions, and support for coordination of care to meet a patient's psychosocial and health care needs.
